  • Stakeholders should represent the communities

  • where services are provided.

  • They can include people with diabetes,

  • health care providers, and members of

  • community-based groups.

  • Their feedback should be gathered and documented

  • within 6 months of DSMES accreditation

  • and then at least every year.

  • Feedback can come from a variety of methods,

  • like email,

  • face-to-face meetings,

  • conference calls,

  • and surveys.

  • Ongoing stakeholder feedback is an

  • essential element for

  • strong DSMES services.
