  • requires you to be able to

  • analyze outcomes and evaluate your

  • DSMES service's effectiveness.

  • Process, clinical, and behavioral outcomes

  • can all be measured.

  • Diverse criteria such as

  • number of referrals,

  • A1c levels,

  • and behavioral goal achievement

  • are just a sample of

  • the measures that can be used.

  • Having these data allows

  • the quality coordinator to determine

  • the effectiveness and impact of

  • DSMES services and to implement

  • continuous quality improvement.

  • Data collection and analysis should be

  • completed with each DSMES participant

  • to determine current

  • successful strategies

  • and to identify areas for improvement.

  • Using these data,

  • the DSMES quality coordinator

  • can objectively see how

  • the DSMES service is performing

  • and establish a plan

  • for improving the service.
